Our practice is out of network at this time. Clients can pay out of pocket and then submit for out of network reimbursement.
If you will be submitting for out of network reimbursement from your health insurance, please check on the following before your first appointment by logging into your insurance plan’s website or calling the number on the back of your insurance card:
- Do I have out of network benefits? If yes, how do I submit for reimbursement?
- What is my responsibility or co-payment for outpatient mental health services? Please note that the co-pay you have printed on the front of your insurance card often differs from your mental health copay.
- Do I need pre-authorization for outpatient mental health services? How many sessions does my insurance plan allow? Please provide provider name (Melissa Laracuenta,PsyD) and NPI (National Provider Identification) is 1285891390 if necessary.
- Do I have a deductible? And if so, how much of it have I used?
All fees are due at time of service and can be paid via cash or credit card.
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
